Office Appointments with Northern California Neurosurgery

Patients are seen by appointment only. We can see new patients in a week or less.    If you need to cancel an appointment, please give as much notice as possible.  For your first visit, we ask that you come fifteen minutes early to complete a brief questionnaire.

Preparing For Your Visit: 

Xray of spineTo ensure that you get the very best medical care, our doctors will need to review all of your medical records, X-rays and reports. Please bring a list of all of your doctors, a list of all of your surgeries and procedures, and a list of your current medications. Please try to get copies of your X-rays before the visit.  We need to see the films themselves.  If you cannot get the films, please try to get the reports,. The intake forms can be downloaded from this web site and completed in advance of the visit.  If you can, bring the completed forms to the visit.

Forms to complete before your neurosurgical appointmentThe receptionist can also help you locate any documents not in your possession before the visit.

It is often helpful to write down your questions in advance. We save time to answer your questions. 

Finally, please bring all of your insurance cards to each visit.
